Field Trip Information Form

To register your school group, finalize the booking of your field trip and to be sure you are on the schedule, please fill out the following information completely. All fields are required.
It is highly recommended that you print this and give to each teacher to read and be aware of the acknowledgements you have agreed to This way they can pass along to parents as well.

All acknowledgements must be checked. We recommend printing a copy for your other teachers to review.

I acknowledge that the standard arrival time for field trip is 9:30. I will notify Sweet Season Farms if we cannot make this time or if we are running late on morning of field trip. I understand that a late arrival does not mean a later departure time.

I acknowledge that departure time for field trip is no later than 12:00 for all students, teachers and parents. I will make sure all grade level teachers know this as well as inform parents in letters sent home. Farm closes at 12:00.

I acknowledge that it may be HOT on the day of our field trip and teachers will make arrangements to have extra water for each student. I will also pass this information along to each teacher/class attending the field trip. No water fountains at the farm, but water bottles are available for purchase. Sweet Season Farms cannot be responsible for providing cups of water for your students.

I acknowledge this field trip is to a farm and all activites will be OUTDOORS. Closed toed shoes are best for walking at farm. I will pass that information along to all other teachers and parents attending the field trip.

I acknowledge that the field trip price is $11.00 per student and will be paid with cash, school check or school credit card on the day of the field trip at check-in. Grade level Teachers and Bus Driver are free. Teachers from other schools or grades attending are considered parents and must pay upon arrival.

I acknowledge that Parents and Chaperones attending field trip are $10.00 each and they will pay at the farm UPON ARRIVAL. (Please do not collect parent or sibling fees. If your school requires this, please email us ASAP) If siblings come with parents or chaperones, they will be charged $10.00 each for ages 2 and older and must stay with the assigned group to participate. I will pass this information along to all other teachers and parents. Also, I will pass along to parents that the field trip is geared toward the age of the field trip student. Younger or older siblings may not be able to participate in all activities. They will be required to stay with group at all times and not allowed to roam to unopened weekend only activities. NO EXCEPTIONS WILL BE MADE

I acknowledge that this is a STRUCTURED field trip and that we will be put into groups and rotate on a time schedule. I will notify all other teachers and parents that we will stay with our designated group throughout the entire field trip. If parents have siblings with them, they will still have to stay with the group as playground activities are not open during rotation time.
