Field Trip Information Form

To register your school group, finalize the booking of your field trip and to be sure you are on the schedule, please fill out the following information completely.  All fields are required
**It is highly recommended that you print this and give to each teacher to read and be aware of the acknowledgements you have agreed to** This way they can pass along to parents as well.

All acknowledgements must be checked.  We recommend printing a copy for your other teachers to review.

Thank you for submitting!  Once this is received, we will email you a confirmation as well as permission slip attachments and more information.  We will also reach back out to you closer to your field trip date, but feel free to email info@sweetseasonfarms.com at any time with any additional questions that may come up!   See you at the farm!
