Field Trip Information Form
To register your school group, finalize the booking of your field trip and to be sure you are on the schedule, please fill out the following information completely. All fields are required.
All acknowledgements must be checked
I acknowledge that the standard arrival time for field trip is 9:30. I will notify Sweet Season Farms if we cannot make this time or if we are running late on morning of field trip. I understand that a late arrival does not mean a later departure time.
I acknowledge that departure time for field trip is no later than 12:00 for all students, teachers and parents.
I acknowledge that it may be HOT on the day of our field trip and I will make arrangements to have extra water for each student. I will also pass this information along to each teacher/class attending the field trip that day.
I acknowledge this field trip is to a farm and all activites will be OUTDOORS. Closed toed shoes are best for walking at farm. I will pass that information along to all other teachers and parents attending the field trip.
I acknowledge that the field trip price is $7.50 per student and will be paid with cash, school check or school credit card on the day of the field trip at check-in. Teachers and Bus Driver are free.
I acknowledge that Parents and Chaperones attending field trip are $5 each and they will pay at the farm UPON ARRIVAL. (Please do not collect parent or sibling fees) If siblings come with parents or chaperones, they will be charged $7.50 and are able to do activities with the group. I will pass this information along to all other teachers and parents.
I acknowledge that this is a STRUCTURED field trip and that we will be put into groups and rotate on a time schedule. I will notify all other teachers and parents that we will stay with our designated group throughout the entire field trip.
